    How do I fill in the interactive PDF?
    Open the PDF and select a score for each statement. Then, save the PDF so that your entries are preserved.
    What if the automatic scorekeeping doesn't work?
    Not every browser or PDF reader supports automatic calculation. You can also add up the scores yourself: for each theme, sum the scores from the four statements. Each statement is worth 0 to 3 points; you can earn a maximum of 12 points per theme. For example: 2 + 3 + 1 + 2 = 8 points.
    Do I need to know all the answers already?
    No. Not knowing whether something has been set up is also a valuable outcome. Make a note of your open questions and determine with your colleagues what further substantiation or verification is required.
    Will my answers be shared automatically?
    No. You fill in the PDF yourself and keep your answers in your own files. We do not see your completed report. We can only view your answers if you choose to share the completed scan with us via email. In that case, we will get in touch with you.
    What can I do after filling it out?
    You can begin working with these initial insights yourself, together with your colleagues and managers. Discuss the findings and select up to three initial actions. You may also email your completed scan to us, and we will then get in touch with you. Please do not include any patient data or sensitive network details.
